One of the primary reasons for industrial accidents is cutting corners in the belief they serve no purpose. The Captain of the Titanic E J Smith – and Francesco Schettino of the Costa Concordia – both sailed dangerously close to deadly objects.

When journalists interviewed the Titanic Captain prior to the maiden voyage he said, “I will say that I cannot imagine any condition which could cause a ship to founder. I cannot conceive of any vital disaster happening to this vessel. Modern shipbuilding has gone beyond that.” That assumed he was prudent.

The Costa Concordia captain famously claimed the rock was not on his chart. The chart existed, although he did not have it in his chart-room because it was off his official route – as were many other places from Cape Town to Cairo for example. The rock does however appear on every tourist map of Giglio Island. The two captains had two things in common. These were a cavalier attitude, and a propensity to show off.
